1.  Date, Name and Member ID

2.  Type of Game, County of the harvest, Photo of your face with your member card and your animal.

3.  If you are submitting a Turkey, please include a photo of tape measure of the beard and spurs with your member card. 

3. If you are submitting a deer over 125'' please include a copy of an official certified WBBC, P&Y, B&C score sheet, please include the contact information for the measurer.
